INSTRUCTIONS AND GUIDELINES
PLEASE BRING ALL COMPLETED REGISTRATION FORMS WITH YOU TO YOUR APPOINTMENT
• Student(s) must be present with the parent/guardian at the time of
registration
• All Information MUST BE PRINTED in INK
• ALL FORMS, including the Physician Form with attached immunization
records and proof of physical, MUST BE completely filled out and signed
by the doctor before appointment
• ANY MISSING DOCUMENTS WILL RESULT IN THE NEED TO
RESCHEDULE YOUR APPOINTMENT
Elementary, Middle, and High School
• Follow directions above and Call (201) 392-3629/3639 for Appointment
• Registration for the upcoming school year begins last week of June
Early Childhood Abbott Program
• Follow directions above and Call (201) 392-3677 for Appointment
• Children must be 3, 4, or 5 years of age before September 1
• It is recommended that your child be toilet-trained
• Registration for the upcoming school year begins the last week in April
• Student(s) may also be registered directly at any participating Center (for that
Center only) * Admittance is subject to Availability
